How to Apply for duplicate Mark-sheet from Kalina university(Mumbai University)

Step to obtain the lost/misplaced mark-sheet from university of mumbai Kalina ☺

Documents you need to get first are :- 

a). Affidavit On Rs. 100 Stamp paper and get it Notarized.(Notary may cost you 100 bucks)
b). First Information Report (FIR) from police station.
c). Write a Letter to " The Controller of Examinations ".

After Getting All your Document follow these steps :-
1). Visiting University :
There are 2 ways to reach kalina university via

Kurla : For People who Use Central Railways.
1) Auto Rickshaw will take you to Kalina University from Kurla Station Fare of Rs. 20 (Yr 2015)
2) BEST BUS NO. 318 VIDYANAGRI  . Will take you at the Fare of Rs. 10 (yr. 2015) Near MAHATMA PHULE BHAVAN , Kalina University.

Santacruz : For People who Use Western Railways.
1) Auto Same As Said Above.
2) BEST BUS NO. 313  will take you to kalina university.

Timing :
Monday to Saturday - 9:30 am to 5 pm
Lunch Break : 1:00 pm to 1:30 pm (but they open the window at 1 : 45 pm or 2 : 00 pm)
Fee Payment time is till 2: 00 pm
If you want to apply for duplicate mark-sheet please reach there before 1 o'clock.

2). Once you'r there. Bingo!!

Stand in the queue near Window No. 2.
Show him your documents (Affidavit , FIR and the Letter)
Then He Will give you Small form. Fill that Form,and Get permission from Window no. 1 to Enter the building , Where you need to take Sign of " Controller of Examinations ", from ROOM NO. 24A on 2^rd Floor of MAHATMA PHULE BHAVAN .

Once you get the Sign, Go to 3^rd Floor to Pay Rs. 100 as fine.

Then He will Give you the Receipt and you need to wait 15 to 20 days Max. to get your Mark-sheet.

Detail about each documents.

a). Affidavit : -

Copy and paste in MS Word , and fill the blanks. Enter the lost Mark-sheet's SEAT NO. and MONTH AND YEAR. ALSO YOUR COLLEGE CENTRE NO. where you had given your exam. (Mostly its your own college.)

1. That the original Mark-sheets /Diploma in course …………………………………….. From……………………………………………………………, Maharashatra in the year of …… to ……, Have been misplaced / lost ……(location ,eg. In bus etc)……………..(date and time)………..

2. That my Seat. No……………. for the said Mark-sheet/Diploma.

3. That if the said Mark-sheet / Diploma are found in future I will submit the same to the
concerned authority.

4. That I will not misuse the said Mark- sheet / Diploma.

5. That I request to the concerned authority to please issue me the duplicate Mark-sheet / Diploma.

6. That the above said contents are true.

b). FIR :-

You need to register your FIR in the same City where you lost your MARK-SHEET.

ALSO YOU WILL BE NEEDING

1). MARK-SHEET Xerox is needed while Registering A FIR, In case you don't have it than its OK! They Will accept you Complaint.
2). CARRY YOUR ADDRESS PROOF like AADHAR CARD OR ANY.
3). XEROX of your AFFIDAVIT.

c). Letter to Controller of Examinations
